Server down?!?!

Is this still happening? How often does it happen? I've updated to the latest version of sourcemod the other day, but it seems like something else is going on.
 
I've noticed the server crashing in the server uptime, but nothing in the error logs show what is causing this. The only thing I can think of is the chat colors, that's the only addon I recently added like a month or two ago.
How long has this been going on for?
 
We have enabled -condebug. Next time the server crashes, we will hopefully have more data.

I m not sure if this would help, but recently, when every time someone joined the server I got a serious lag with my computer, its like the win98/XP CPU overloaded lagging. The sound keeps looping itself in a super high frequency, and I can't do anything with my computer(not only the game.) All I can do is to wait for it to stop lagging, usually it will only lag for seconds but sometimes longer. I do not know what is causing this problem but this drives me crazy especially when I was shooting someone or not in a safe place. Most of the time this would get me killed.

edit: my hard drive is too slow, I think it might cause this problem.
 
The above has nothing to do with the server. Also you should post a tracert on a new thread.

Sounds like a swap file issue to me.
